Once you arrive to your appointment, you are welcome to relax in our patient waiting area. Mask requirements may vary depending on local community COVID-19 levels. If you would prefer to wait in your vehicle, please call or text the office letting us know what vehicle you are in, and your provider will escort you directly to your appointment chair.

New Patients
Please complete your new patient forms at home.
Once you follow the above link, select New Patient and scroll down to select a packet to complete. No need to print anything out.
Our online paperwork is preferred and encouraged. Using our online portal is the easiest and fastest way to complete your forms.

If you would rather complete printed paperwork, printable patient forms may be downloaded below. Completed forms can be brought in during your visit or emailed to springcreekdds@gmail.com.
